Audrey Allen was born on 29 June 1935 and had previously enjoyed an independent life, with the support of her family, in Chesterfield. When she started displaying symptoms of demenia, she moved to a care home operated by Derbyshire County Council in December 2015. She sadly passed away on 16 April 2016 after sustaining a fall at the care home. The investigations that followed Miss Allen’s death revealed a series of failures in the management of her care. In the Coroner’s view, Miss Allen would not have died when she did had the care home carried out the necessary risk assessments.
