The inquest took place over 5 days at Redhills from 9th to 13th August 2021 and touched on the death of a serving prisoner at HMP Durham, Trevor Green, who died as a result of self-inflicted injuries on 21st July 2020. The inquest examined the assessment and management of risk of self-harm and suicide for new arrivals during the reception and induction process at the prison. The jury returned a narrative conclusion on 13th August 2021 with no criticism of the actions of discipline staff at the prison.
